- Aadhaar Card of Child
- Aadhaar Card of Parent/Guardian
- 4 Passport-size Photographs of the Child
- 2 Passport-size Photographs of Each Parent
- Address Proof (Aadhaar/Utility Bill)
- Previous School Transfer Certificate (if applicable)
